Intensive In-home Services

 

 

 

Specific Services:

 

Conduct comprehensive child and adolescent psychological and family/social history evaluation of treatment needs.

 

Administer weekly individual and family therapy with 24/7 crisis intervention provided as needed (5-10 hours).

 

Referrals and Funding:

 

Referrals may come from the Community Service Board, current outpatient counselor, school system, court services, Family Assessment and Planning (FAP) Team, etc.

 

Medicaid, Comprehensive Services Act (CSA) funding or special grants cover intensive in-home services.

....With Individualized Services....

Families that find themselves in crisis are usually coping with a multitude of issues. We feel that the only way to provide effective treatment is to ensure that treatment is specific to the needs of each family. We feel it is not enough to develop generic goals for treatment based only on a mental health diagnosis. That is why all cases are staffed with a Clinical Supervisor weekly and any time a crisis arises.

 

....With Home and Community Support....

—Although services are based in the home, we believe that treatment must occur where needed. We work to develop improved socialization skills through providing opportunities to utilize and monitor techniques that are taught.

 

....With School Based Support....

—Intensive-In-Home Services will support these efforts by conducting school visits. —Monitoring educational progress to include attending IEP and Child Study meetings—. Monitoring behavioral issues and working to develop the necessary skills to eliminate those issues—. Acting as an advocate and liaison between the school and parent.— Counselors will meet with School Truancy Officers to regularly monitor attendance in an effort to prevent failure to comply issues. Although we seek to assist all students, we feel early intervention is most effective. Truancy issues at the elementary level may be reduced or halted by providing support to the parent.

Youth Solutions provides Intensive-In-Home counseling to families in the greater Hampton Roads area. Our qualified mental health professionals assist client’s in their home where the need is the greatest. A client-based Individual Service Plan is created and utilized to assist clients in meeting agreed upon goals and objectives. From the onset of service, our goal is a successful discharge where clients are no longer at risk of being removed from their home.

 

Intensive-In-Home services consist of family-focused, strength-based family therapy in the home and community for children/adolescents with serious emotional disturbances, several behavioral disorders, or substance abuse problems. A counselor works closely with all involved systems (social services, schools, court services, health care providers, etc.) to provide the appropriate assistance. 

 

Intensive-In-Home Service Eligibility

 

Intensive-In-Home services are for those who:

  • Meet the criteria for serious emotional disturbance; or

  • Are at risk of out-of-home or out-of-community placement; or

  • Require more intensive therapy than outpatient therapy can provide; or

  • Are transitioning back to the home (biological or foster) from an out-of-home placement
