We provide mental health skill-building services for adults who receive Medicaid benefits, have had a prior psychiatric hospitalization and are prescribed psychotropic medications. These services include goal directed training to help individuals achieve and maintain community stability and independence.  Training is provided in the following areas; activities of daily living, use of community resources, assistance with medication management, and assistance monitoring health, nutrition and physical condition and needs.

Adult Mental Health Skill-Building Services

The Individual Seeking Adult Mental Health Skill-Building Services...

....must have a primary Axis I DSM diagnosis of; Schizophrenia or other psychotic disorder, Major Depressive Disorder, Bipolar I or II, or any other Axis I mental health disorder that a physician has documented.

 

....shall have a prior history of any of the following; psychiatric hospitalization, residential crisis stabilization, Intensive Community Treatment (ICT), placement in a psychiatric residential treatment facility (RTC Level C), or Temporary Detention order (TDO) evaluation.

 

....shall have had a prescription for a psychotropic medication

Specific Services:

  • Goal-directed training to help individuals achieve and maintain community stability and independence.

  •  Instruction in completing and maintaining activities of daily living, such as;  personal hygiene, food preparation, and money management

  • Assistance with locating and securing community resources.

  • Assistance with maintaining psychiatric treatment plans and medication management, to ensure medication adherence.

  • Assistance monitoring health, nutrition and physical condition and needs.

  • Instruction on symptom management.

  • Assistance in developing appropriate social skills, and the proper utilization of their personal support system
