1) Referral 

• A student is identified as at risk

• A referral form is completed and submitted to Youth Solutions Inc. (this may be done via email or fax)

• Youth Solutions Program Director contacts the family to explain the services, the process for initiating, and offer support in completing the VICAP process

• After the VICAP is completed the information is presented for Medicaid authorization

 

 2) Eligibility 

• ​ Client Meets Eligibility Criteria

 

​3) Authorization

• Medicaid Authorization is received usually within 1 week of submission

4) Assignment

• Once services have been approved, clients are assigned a counselor within 24 hours and services are initiated. The assigned counselor will continue to assess the specific needs of the family while addressing immediate stabilization needs during the first thirty days of service.
